Rate sustainability mechanisms

Rate each of the following sustainability mechanisms to indicate how well they support the long-term maintenance of the PSE change:

  • Organizational Responsibility — A specific organization or team is responsible for maintaining the PSE change.

  • Funding — Dedicated funding exists to sustain the PSE effort over time.

  • Policies — Formal policies are in place to support the continuation of the PSE change.

  • Reporting — Regular reporting or monitoring mechanisms exist to track the status of the PSE change.

  • Stakeholders — Key stakeholders remain engaged and committed to sustaining the effort.

  • Other — Any additional mechanisms that support sustainability.

Select policy levels

Select the Policy Levels at which policies supporting the PSE change exist. Multiple levels may be selected to reflect policies at different scopes (e.g., local, state, federal).

Select stakeholder groups

Select the Stakeholder Groups that are engaged in sustaining the PSE change. Identify all groups that play a role in the long-term success of the effort.

Enter assessment results

Enter the Assessment Results that document the current state of sustainability for the PSE change. Include any findings from sustainability evaluations or reviews.
