Readiness

Document readiness assessments performed for a PSE Site Activity, including assessment tools, dates, results, scores, and follow-up information.

The Readiness section captures whether a readiness assessment was conducted to evaluate the site's preparedness for PSE changes. If an assessment was performed, you can record the tool used, results, and any follow-up assessments.

Indicate if readiness assessments were performed

Select Yes or No to indicate whether any readiness assessments were performed for this PSE Site Activity.

If you select No, no additional readiness fields will be displayed. You may proceed to the next section.

Add an assessment tool type

Select the Assessment Tool Type used to evaluate readiness at the site.

Select the assessment tool

Select the specific Survey Instrument from the available options based on the tool type selected.

Enter the assessment date

Enter the Date when the readiness assessment was conducted.

Enter the assessment results

Enter the Results of the readiness assessment, describing the findings and any observations.

Enter the assessment score

Enter the Score received from the readiness assessment, if applicable.

Enter follow-up date

Enter the Follow-Up Date for any subsequent readiness assessment planned at the site.

Enter follow-up results

Enter the Follow-Up Results from the subsequent assessment, describing any changes observed.

Enter follow-up score

Enter the Follow-Up Score received from the subsequent readiness assessment.

Multiple readiness assessments can be added to track progress over time. Click Add Another Assessment to record additional assessments.
